Referral form for teachers

As a teacher you may be faced with two different scenarios involving drug or alcohol and a young person in your class and school.

One
If a young person your school has been caught with substances, for advise and help with mediation call

Two
If a young person needs one to one support relating to their drug or alcohol use, refer that young person to the single point of contact for all schools, Melanie Clarke. Melanie will allocate a drugs worker who will see the young person either in school or a youth centre.

Who are we? What do we do?

EVOLVE is a confidential treatment service, which provides community support for 10-18 year olds with complex and serious substance misuse issues in Oxfordshire. We work with young people to effect change around their substance misuse, whilst also addressing related issues and risks. We also offer advice and information to parents and carers.

The team is multi-disciplinary comprising:

The service works from a central base in Oxford and satellite offices around the county. Project workers will also see young people in a range of venues across the county including young peoples homes, GP surgeries, Information Shops, Youth Clubs or other offices we can book.

In addition to our work with young people, we offer consultancy and advice to professionals working with young people.

Who do we work with? How do we work?

The service works in accordance with HAS (Health Advisory Service 2001: The Substance of Young People's Needs). OYPTS provides a service at Tier 3, which targets young people with substance misuse issues, often with multiple underlying problems. The team works with these multiple needs of a young person in a holistic way addressing issues such as mental health, offending behaviour, self-esteem, aggressive behaviour, educational needs and family relationships.

Following referral, clients will undertake a comprehensive assessment to establish their needs. Therapeutic interventions are then planned and agreed with the client using a care-plan approach. The Doctor and Nurse in the team will carry out mental health assessments, when required, and offer prescribing of psychiatric or substitute medication. An intensive community detoxification programme can also be provided by the service following a period of assessment and engagement.

Where appropriate, our service will refer on to other services to support a young person.

How do you refer?

If you would like to make a referral, it is important that the young person is aware of the referral and agrees to their information being passed on to us.

Please complete our referral form with as much detail about the young person's substance misuse as possible. Send or fax the form to us. If you do not have a referral form, please call the office and this can be done over the phone. The multi-disciplinary clinical team hold team meetings every Wednesday, where new referrals are discussed and allocated for assessment.

We are happy to meet with staff or attend team meetings to discuss our service further.

Referral criteria for a Tier 3 substance misuse intervention include:

  • Aged 10-18.
  • Living within Oxfordshire.
  • Problematic and high-risk substance misuse issues, requiring
    • A specific and structured substance misuse intervention (e.g. education, harm minimisation and relapse prevention).
    • Access to substitute prescribing or community detoxification.
    • Screening and advice regarding blood-borne viruses.
    • Auricular acupuncture.

We also accept self-referrals from young people.

If you are unsure whether a client you would like to refer is appropriate for the service, please call and speak to a member of staff. You do not have to provide the client's details to do this. IF IN DOUBT PLEASE CALL US!
